At West Branch, we are committed to providing clear and timely information about your
health coverage and related tax documents.
In accordance with the Affordable Care Act (ACA), Form 1095-C (Employer-Provided
Health Insurance Offer and Coverage) is available upon request for individuals who are
eligible to receive it. Please Note: although individuals are no longer required to
provide this form with their tax documents, your employer is still required to make
it available.
